Most teams have solid outage playbooks — they know exactly what to do when the database goes down or a deploy goes sideways. But ask those same teams what they'd do when suspicious behavior shows up in their PR workflow, and you'll usually get a shrug.
You need a playbook for both. When things get weird in your review process, having a clear structure beats improvising under pressure every single time.
When to trigger incident mode
Here are the signals that should flip your team into a more deliberate response mode:
- Low-confidence PRs keep showing up in short cycles
- Comment threads are getting heated and pressure is escalating
- The behavior looks coordinated across multiple PRs or accounts
- Public posts or social media threads are targeting your active reviewers
To be clear — triggering incident mode isn't a legal judgment or an accusation. It's an operational signal that says: “risk is elevated, let's switch to a tighter process until we understand what's going on.”
The 5-step response
1) Assign an owner
Pick one person to own the incident and one backup. Keep everything in one thread with one decision chain. Nothing creates chaos faster than three people making parallel calls about the same situation.
2) Contain the noise
Keep PR discussion short and grounded in policy. If the conversation gets complex or emotional, move the real debate to an internal channel. The PR thread should stay clean and factual — that's what people will screenshot.
3) Capture the facts
Before anyone forms an opinion, write down what actually happened:
- Timeline of events
- Key comments and interactions
- Risk indicators that triggered the escalation
- Final decision and who approved it
Facts first, opinions later. This log will be invaluable when you debrief.
4) Decide by policy
This is where pre-defined outcomes save you. Your options should already be written down somewhere:
- Continue with normal review
- Escalate to senior or security review
- Close the PR pending policy compliance
The worst thing you can do is invent new rules while you're under pressure. Lean on the policies you already have.
5) Review and improve
Once things settle down, run a quick 20-minute debrief with the team:
- What did we catch late that we should have spotted earlier?
- Which policy text was confusing or hard to apply?
- What part of this could we automate for next time?
Then ship one policy update and one workflow improvement. Small, concrete changes compound over time.
Communication templates
For your internal team
Keep the internal update tight and actionable:
- What's the scope of this incident?
- Who owns the response?
- What temporary rules are in effect?
- When's the next checkpoint?
For external communication
When responding publicly, keep it brief and neutral:
- “We follow our repository review policy for all contributors.”
- “Final merge decisions stay with maintainers.”
That's it. Don't over-explain, don't get drawn into debates. Short and factual wins every time.
A first version you can deploy today
You don't need a perfect playbook to get started. Here's a minimum viable version:
- Write a one-page PR incident runbook — even a rough one is better than nothing.
- Set up an owner rotation so it's always clear who's on point.
- Start logging every escalated review event, even if it's just in a shared doc.
You can refine and improve the playbook over time. What matters right now is having something in place before the next incident catches you off guard.
